Empowering Minds and Development is a registered Specialist Support Coordination team that is committed to implementing strengths-based, trauma-informed, and culturally sensitive practices. The Specialist Support Coordination team based in Brisbane focuses on addressing barriers and reducing complexity in the support environment while assisting our NDIS participants in connecting with support and building capacity and resilience in their communities.

This approach supports a service delivery model where service delivery is planned, integrated, goal-oriented, and accountable while ensuring each participant receives culturally appropriate services in an effective and efficient manner that is consistent throughout the NDIS system.

  • It is funded in the ‘Capacity Building’ support category of your NDIS plan. If it’s not there, you can request a review by the NDIA.

Holistic

We take a holistic and trauma informed approach to delivering two levels of Support Coordination under the NDIS:

  • Support Coordination: ongoing support throughout the duration of your plan, for people with more complex needs who may require additional support to connect with providers.
  • Specialist Support Coordination: higher intensity Support Coordination. Our multidisciplinary team partners with you and your support network to action your NDIS plan and reach your potential.

Director

With qualifications in Human Services, Criminology and Criminal Justice, aboriginal and intercultural studies, and public sector management, Liz has a unique skillset.

Liz is a highly skilled strategist with more than 10 years’ experience in the human services and community sector that began in front line service delivery. Liz experience includes leading practice improvement through the implementation of evidence-based practices and programs with a specific focus of embedding trauma informed practice within the service delivery.

Liz has extensive experience working with complex clients in the criminal justice, child protection and mental health services.

With a strong reputation in the NDIS sector and with Government, Liz is respected for her ability to collaborate and partner in a truly authentic way, and is a prominent and reliable voice on ways to deliver high quality supports and outcomes for clients seeking support and services from the non-government sector.

Liz leads her teams from a values, trauma informed and culture driven approach that seeks to inspire the leadership qualities in others.

Sal Wilson

Operations Manager

Sal is a community focused, results driven and accomplished professional and encompasses over 17 years working in the human services sector, including experience overseeing the program management office leading Queensland’s transition to the NDIS, providing assurance, consulting and risk management services.

Sal’s academic background and employment within the government sector has afforded her an understanding of the necessity for evidence-based practice and knowledge of theoretical perspectives that relate to delivering best practice services. She has exceeded expectations with implementing evidence-based practice and conducting evaluations both in the delivery of specialist support coordination services and business development.

Sal’s therapeutic and operational experience, in conjunction with her professional framework, endless energy and unshakable resilience gives her the knowledge and expertise to lead our operations in Queensland.

Our team provides allied health services, including Occupational Therapy, Speech Therapy, Social Work, and Physiotherapy. Our NDIS functional assessments evaluate your abilities in daily life tasks across various settings, focusing on well-being, independence, support needs, mobility, communication, self-management, learning, and self-care. Our detailed yet simple NDIS functional assessment reports, conducted by experienced clinicians, assist in aligning NDIS funding with your goals and improving your quality of life.

Early Intervention

Early Intervention

The Early Childhood Intervention program is a national program supporting children aged 0-9 who have a developmental delay or disability and their families, funded through The National Disability Insurance Scheme (NDIS).
Child Development is supported by the Early Childhood Approach Scheme and services can involve prevention, early intervention, literacy support, referral to specialist support, case management and home visits.

EMD Steps To Implementing Your NDIS Plan:

  • Initial

    Connect and communicate with client /plan nominees

    Review the plan, gather information to understand current supports identify additional (if required)

    Sign NDIS Consent forms

    Sign the Privacy & Confidentiality form

    Complete the Individual Client assessment form

    Arrange services

  • 6-8 week report

    Brief progress report and list of services connected or to be arranged

    Get feedback from participant, families, and carers.

    Action allied health report recommendations

  • End of plan report

    Meet participant and plan nominee to prepare

    Review the current plan period progress to adjust the next plan

    Gather other health professions report and summaries

    Write end-of-plan period report and provide recommendations for the next plan

    Get feedback from participant, families, and carers.
